my insert is totally clipped up so how do I get the rebate form?
@kellis The rebate form didn’t have coupons on the other side of it, so it shouldn’t be clipped up. The only way to get this rebate form that I know of at this time is to have gotten it in the insert. Some areas did not receive the insert, hopefully yours did.
Anyone out there have an extra rebate form? I’d love to get a copy & could trade coupons, especially baby or pet coupons, as I don’t have either.

Holly I got my rebate already. It came 2 days ago. Rebates usually take 4-6 weeks to arrive.
